Indicate the recommended vaccinations for your patients by placing your initials in the “Provider initials” column, along with the recommended timing in the “Recommended vaccination date(s)” column.

Direct patients to scan the EasyVax QR code on the Patient Vaccination Log to schedule their vaccinations.

At each visit, review and update your patients’ vaccination status in the “Date(s) administered” column.

Place a checkmark in the “Status” column upon completion of vaccination(s) and discuss which other vaccines may be appropriate for your patients.
